Cheapest Tirzepatide Online in 2026: Verified Prices at Every Dose

Ranked by what you actually spend over twelve months, not by the starter-dose price on the homepage.

2.5 mg 5 mg 7.5 mg 10 mg 12.5 mg 15 mg

Short answer

The lowest verified price for compounded tirzepatide is $199 per month from Oak Longevity, followed by NexLife at $215, bmiMD at $233, Join Fridays at $249. All of these dispense through US-licensed compounding pharmacies with prescriber review.

But the cheapest advertised price is frequently not the cheapest year. Programs that scale price with dose can more than double by the time you reach 10 mg. Ranked by twelve-month cost on a realistic escalation curve, the order changes: Oak Longevity ($2,388), NexLife ($2,580), bmiMD ($2,796).

For FDA-approved product, the cheapest route is LillyDirect self-pay Zepbound vials at $299–$449 per month depending on strength.

Ranked by twelve-month cost

Every row below assumes the escalation most people actually follow: four weeks at 2.5 mg, four weeks at 5 mg, then ten months at 10 mg. That is the shape of a first year on tirzepatide, and it is the only fair way to compare a flat-rate program against a dose-scaled one.

What "verified" means here

Every price on this site carries one of three verification levels, and they are not the same claim. 14 records are verified at source; 9 carry a third-party figure we have not confirmed with the provider; the rest publish no price at all.

  • verified at source — Price read from the provider's own site, the manufacturer, or a named editorial source with a stated verification date.
  • third-party figure — Price recorded from a third-party review supplied to us, not confirmed against the provider's own published material. Treat as indicative and confirm before enrolling.
  • no price published — No figure we could interpret or verify. We publish nothing rather than estimate.

Levels are published per record in the open dataset. Where we verified a figure directly, seven times out of seven the correction changed the number.

Monthly price by dose tier — all-in monthly price (medication plus any membership fee) at each dose tier, verified 2026-08-05.

Why the cheapest advertised price is usually not the cheapest

Four structures inflate the real number, and all four are legal and disclosed somewhere in the terms:

Dose-scaled pricing. The advertised figure applies to 2.5 mg — a dose you occupy for four weeks. Your prescriber escalates because that is what produces results, and the invoice follows. By 10 mg some programs have more than doubled.

Membership stacking. A competitive medication price plus a separate recurring platform or coaching fee. A $199 medication with a $79 membership is a $278 program. We add these automatically.

Introductory rates that revert. A low first-cycle price that returns to standard at renewal. Ask specifically whether the quoted rate holds through the full commitment.

Unbundled fees. Consultation, shipping, lab work or dose-change charges billed separately. Most tracked programs quote all-in; not all do.

Why nothing legitimate costs under about $99

Active pharmaceutical ingredient, sterile compounding under USP standards, potency and sterility testing, refrigerated shipping and a prescriber's time all have real costs. Below roughly $99 per month, something in that chain is being skipped. The usual candidate is the provenance of the active ingredient.

Products advertised as "research use only" are not manufactured to pharmaceutical standards and are not legal to sell for human use. See our counterfeit guide for the full set of tells.

The FDA-approved options, for comparison

  • LillyDirect self-pay Zepbound vials — $299/mo at 2.5 mg, $399 at 5 mg, $449 at 7.5 mg and above with a refill inside 45 days. No subscription; each fill is its own order.
  • Zepbound Savings Card — as low as $25/mo, but only with commercial insurance that covers Zepbound for weight management. Unavailable on Medicare or Medicaid by federal law.
  • Mounjaro Savings Card — same molecule, $25/mo with commercial insurance and a type 2 diabetes diagnosis.
  • Medicare GLP-1 Bridge — $50/mo since 1 July 2026 for eligible Part D enrollees who meet the BMI and comorbidity tiers.
  • Retail Zepbound — around $1,089/mo with no program applied.

Before you pay, get these in writing

  1. The monthly price at 10 mg and at 15 mg — not the starter price.
  2. Whether any membership, consultation, shipping or dose-change fee applies.
  3. Whether the quoted rate holds at renewal.
  4. Which pharmacy compounds your medication, by name.
  5. What happens if a cold-chain shipment arrives warm.
  6. Refund terms if you stop after one month of a prepaid plan.

Frequently asked questions

How much does tirzepatide cost per month without insurance?

Compounded tirzepatide from licensed US telehealth programs runs roughly $99 to $299 per month depending on the provider and whether the price is flat or scales with your dose. Brand-name Zepbound through LillyDirect self-pay runs about $299 to $449 per month depending on strength. Retail Zepbound without any program is around $1,089.

Why do tirzepatide prices go up as my dose increases?

Because some programs price by dose tier rather than charging one rate. The advertised price usually reflects the 2.5 mg starter dose, which you occupy for about four weeks. Flat-rate programs charge the same amount at every dose. Over twelve months that structural difference typically matters more than any first-month discount.

Why is some tirzepatide advertised under $100 a month?

Usually one of four reasons: it is the 2.5 mg starter-tier price only, it is an introductory rate that reverts, there is a separate membership fee, or the product is not from a licensed US pharmacy. Ask what you will pay at 10 mg before you enrol.

Can I use HSA or FSA funds for tirzepatide?

Generally yes for prescribed weight-management medication, including compounded products, though some plan administrators require a letter of medical necessity. Confirm with your administrator and keep every receipt.

Is compounded tirzepatide the same as Zepbound?

The active ingredient is the same molecule. Zepbound is FDA-approved, manufactured by Eli Lilly and reviewed by FDA for safety, effectiveness and quality. Compounded tirzepatide is prepared by a state-licensed pharmacy for an individual patient and is not FDA-approved or FDA-reviewed. The pharmacology is identical; the quality assurance structure is not.

People also ask about cheapest tirzepatide

What is cheapest tirzepatide?

Cheapest tirzepatide is covered in full on this page, with the clinical detail drawn from the tirzepatide prescribing information and the published SURMOUNT and SURPASS trial program, and any price figures drawn from our tracked dataset and dated to their last verification.

Does cheapest tirzepatide apply to compounded tirzepatide as well as Zepbound?

The molecule is the same, so the clinical content applies to both. What differs is regulatory status, dose metering and price: compounded preparations are not FDA-approved, arrive as a multi-dose vial you measure yourself, and cost a fraction of the brand routes.

How much does cheapest tirzepatide cost per month in 2026?

Compounded tirzepatide runs from $133 per month at the entry dose across the 21 compounded programs we track. At a 10 mg maintenance dose the cheapest tracked route is Oak Longevity at $199 per month, or about $2,388 for a first year on the standard escalation. Brand Zepbound through manufacturer self-pay sits well above that and pharmacy retail well above again. Verified 2026-08-05.

Is cheapest tirzepatide FDA-approved?

Brand tirzepatide (Zepbound and Mounjaro) is FDA-approved. Compounded tirzepatide is not: FDA does not approve compounded drugs and does not review them for safety, effectiveness or quality before they are marketed. The active molecule is the same; the regulatory review is not.

Cheapest program that will tell you which pharmacy fills your prescription

The cheapest programs in the table above mostly will not name the pharmacy that compounds your medication. That is the single most useful thing a program can disclose, because it is the one fact that lets you check a state board register yourself before you inject anything weekly. Fewer than a fifth of the priced programs we track publish it.

Ranked among only those that do, the order looks different — and the cheapest option changes.

This is not a safety rating. A program can name six pharmacies and ship late; another can name none and run a careful operation. What naming buys you is the ability to verify, which matters most in the market where verification is hardest.

The same ranking using only verified prices

Most rankings in this market — including the one above — mix figures of very different quality. Some come from a provider's own published pricing; most come from third-party reviews nobody re-checked. Every time we have verified one of those third-party figures at source, the number moved, and usually upward.

So here is the ranking restricted to prices we read at the provider, the manufacturer, or a named editorial source with a stated verification date. It is a shorter table and a more honest one. If a program you are considering is missing from it, that is not a criticism of the program — it means we have not confirmed its price yet, and you should confirm it yourself before enrolling.

Cheapest tirzepatide on a prepaid twelve-month commitment

The ranking above is month-to-month, which is the right basis if you want to keep the option to stop. It is not the right basis if you already know you will be treating for a year. Several programs publish a lower rate for prepaying a term, and the order changes when you rank on that instead — so here is the same market on the other basis.

Read this table with the commitment in view. A prepaid term is money you generally cannot recover once medication ships, and it is the wrong structure during titration when the main reason to stop is a tolerance problem you have not met yet. It is a reasonable structure once you have held a maintenance dose for a cycle. Only programs that publish a prepaid rate appear here; the rest have nothing to rank.

Twelve-month cost if you prepay the published term, ranked. This is a different question from the month-to-month ranking and frequently produces a different answer. Verified 2026-08-05.
