Short answer

Tirzepatide is prescribed at six strengths — 2.5, 5, 7.5, 10, 12.5 and 15 mg once weekly — with a minimum of four weeks at each before stepping up. The 2.5 mg tier is a tolerance-building dose, not a treatment dose. Most people settle between 7.5 mg and 12.5 mg.

In SURMOUNT-1, mean 72-week loss was 15.0% at 5 mg, 19.5% at 10 mg and 20.9% at 15 mg. The gain from 10 mg to 15 mg is small while side effects — and, on dose-scaled programs, cost — keep rising.
