A route that did not exist for the lowest-income patients — but it is optional, so it will be a patchwork.
The federal agreement announced alongside the Medicare GLP-1 Bridge gives states the ability to expand access to Zepbound and the new oral agent through Medicaid. The word doing the work is ability: this is an option states may take up, not a requirement.
That means access will vary by state in the same way Medicaid expansion did, and the states least likely to opt in are frequently those with the highest obesity prevalence. Anyone relying on this should check their own state's programme rather than the federal announcement.
For patients currently paying cash while on Medicaid, this is the first realistic prospect of coverage for a weight-management GLP-1. It is worth asking a prescriber to check status at each renewal rather than assuming the answer has not changed.
Compounded preparations are unaffected. No Medicaid programme covers them, and none of these changes alters that.
