The first drug approved for moderate-to-severe OSA in this population — and a route to coverage that a BMI threshold does not open.

FDA approved Zepbound for moderate-to-severe obstructive sleep apnoea in adults with obesity in December 2024, on the strength of SURMOUNT-OSA, which showed large reductions in apnoea-hypopnoea index at 52 weeks both with and without CPAP.

For coverage this is more consequential than it looks. A documented OSA diagnosis is a discrete medical condition with an approved indication, which is a materially stronger prior-authorisation argument than a BMI threshold plus a comorbidity.

If you have suspected sleep apnoea and are paying cash for tirzepatide, a sleep study is the highest-value thing you can do about your bill.

It does not replace CPAP by default. Reduction in AHI is not the same as resolution, and that decision belongs to the clinician managing your sleep medicine, ideally after a repeat study.
